Medical Transportation Insurance Checklist

Whether you're starting a new medical transportation operation or reviewing existing coverage, this checklist ensures you're fully protected and compliant with your Medicaid broker, state regulator, and local permit office. Use this before every renewal and every new contract enrollment.

Auto Liability — The Foundation

Commercial auto liability is the core of any medical transportation policy. Minimum: $1M CSL for Medicaid broker contracts. Recommended: $1M for standard NEMT; $5M for stretcher or bariatric transport. Always confirm the limit required by your specific managed care contract — some require $2M or more.

General & Professional Liability

Most Medicaid brokers require $1M general liability AND $1M professional liability (errors and omissions). These cover bodily injury and property damage NOT caused by a vehicle, plus claims of improper care, missed appointments causing medical harm, or failure to follow transport protocols.

Abuse and Molestation

Every Medicaid broker — Modivcare, MTM, Veyo, Access Services, and every state MCO — requires abuse and molestation coverage. Minimum $1M. This is non-negotiable. We include it standard on every medical transportation policy we write.

Workers' Compensation

Required from your first employee in most states. Rates for medical transportation drivers run $4–$8 per $100 of payroll. Do not classify employees as 1099 contractors to avoid workers' comp — this is the most commonly audited area in NEMT and the penalties are severe.
