Average NEMT Insurance Cost by Vehicle Type
Sedan/minivan (non-wheelchair): $5,500–$9,000/year for $1M auto liability. Wheelchair-accessible van: $7,500–$13,500/year. Stretcher van: $9,000–$16,000/year. Sprinter 15-passenger: $10,000–$18,000/year. These ranges reflect $1M auto liability + $1M general liability + abuse and molestation. Workers' comp is separate and runs $4–$8 per $100 of payroll for drivers.
NEMT Coverage Requirements by State
Most states require $1M combined single limit for Medicaid contracts. California (Medi-Cal) requires $1M minimum; Florida (SMMC) requires $1M + PIP/PDL; Texas (TMHP) requires $1M; New York Medicaid requires $1M + SUM. Some larger managed care contracts require $2M or $5M — particularly for stretcher transport and contracts that include hospital discharge transfers.
NEMT Fleet Pricing: How Discounts Work
Single-vehicle operators get the least competitive rates. At 3–5 vehicles, carriers begin offering 'fleet' pricing. At 10+ vehicles, specialized transportation underwriters offer experience-rated programs. At 25+ vehicles, captive programs and risk retention groups become viable for operators with strong loss history.
